🚨 Declare an Incident

When multiple departments are hitting the same issue, STOP. Don't try to fix it alone. One commander, one bridge, one plan.

P1 Kickoff Checklist — run this in the first 5 minutes

    Who to call

    Your first call is always the On-Call Duty Manager. They decide severity and declare the incident. They alone hold authority to halt work across departments.

    Severity Guide

    Four levels. When in doubt, round UP. A P2 that becomes a P1 is recoverable; a P1 triaged as a P2 is not.

    Find your department, find the row that matches what you're seeing. The row tells you what to do and who to escalate to.
